Authors need to register with the journal prior to submitting or, if already registered, can simply log in and begin the process.

You can set yourself up to write a great case report by gathering the information you need starting with patient intakes. Here are a couple intake templates you are free to use and adapt, also with a consent form to have your patients sign. Although your patients' personal health information is always federally protected through HIPAA, it's still best practice to have their informed consent, and necessary for publication in most peer reviewed journals.
